What is included in the membership?
Direct access to your physician via phone (direct line) and email
Wellness/Preventative Care visits/School and Sports Physical
Chronic disease management (such as hypertension, diabetes, hyperlipidemia, asthma, mental health, weight management)
Acute care visits (usually same or next day access)
After hours/weekend access when needed (in office by appointment or by phone, text, or virtual visit)
Included Procedures: ear wax removal, laceration repair, incision and drainage of abscess, skin tag removal, wart/skin lesion destruction, cyst removal, skin lesion removal (pathology fee extra or lab can bill insurance), pap smear (pathology fee extra or lab can bill insurance)
Office urine dip analysis, strep screen, glucose, hemoglobin
In office migraine injections
EKG (electrocardiogram)
Access to negotiated near wholesale generic medications with in office dispensing
Access to negotiated low cost labs (usually less than insurance copay)
Access to negotiated radiology/imaging (in process)

Age Price Per Month
0-19: $25*
20-44: $50
45-64: $75
65+ : $85
Enrollment fee $75 for patients not previously established with Dr. Mehra.
Re-enrollment fee is $200 (if you leave the practice by choice or non-payment and rejoin in the future if membership available)
*age 0-19 without parent/guardian enrolled, membership is $40 monthly
Billing and Payment Policies for LakeshoreMD
Enrollment Fee
A one-time, non-refundable $75 enrollment fee will be charged to each new billing account. This fee is waived for patients who were previously seeing Dr. Mehra
Membership Fees
Membership fee(s) are due on the day of enrollment and are prorated to the first of the month. Afterwards, membership fees shall be due on the first day each month for the duration of the membership. Timely payment is required to remain an active member and receive service. Memberships may be cancelled any time with a 30 day written notice.
ALL payment shall be through electronic payment via Auto-Pay with a Credit Card or pin-less Debit Card.
Cancellation
Monthly memberships may be canceled, with a written 30 day notice. All charges, including membership fees, will continue to accrue until notice of cancellation is given in writing.
Re-Enrollment
If a member discontinues membership – by choice or lack of payment – the member may rejoin LakeshoreMD on a space available basis, subject to a re-enrollment fee of $200 per member.
Service Fees
Optional services such as prescriptions, non-covered labs, etc., will be added to the next month’s autopay bill.
